The difference, as it was explained to me, is that water is a natural resource so has no development cost; whereas, insulin and other drugs needed time/money intensive research and development, so the ongoing cost is a partial recouping of the upfront cost of the first dose. Which isn't actually the worst of logic.
The problem is the idea that money is the only incentive for doing work that will pay off later; so the solution is obviously to annihilate capitalism.
Last I checked, most medications I'm aware of cost about $380 to $1200 per KILOGRAM. Chemists in China have an amazing supply chain. Students here with access to chemistry labs confirmed the purity.
